Advance Directives
CHKD understands that you are a unique individual. So are your wishes for the care you want to receive when you cannot speak for yourself.
We're Here to Help
Our Chaplaincy team helps patients and community members -- 18 and older -- understand their options and complete an advance directive, a legal document stating their wishes about how they would like to be cared for should they become incapacitated and unable to speak for themselves.
Chaplains assist patients and community members prepare their wishes on paper so that their family and doctors know how they would like to be treated if they become too sick to tell them.
Please contact a CHKD chaplain to discuss and prepare your advance directive by calling (757) 668-5381.
