Thank You For 35 Wonderful Years!

We would like to thank you very much for the trust you’ve given us at Pediatric Affiliates of Hampton Roads in the last 35 years as your child’s physician. As you know, as of January 1, 2025, we no longer are offering appointments for children. We suggest that you find a doctor in your area who takes your insurance.  We and our staff have truly enjoyed our time seeing your child(ren) grow.  We so valued being with you and your family. For many of you, we have felt very much like a part of your family for many decades.

We are still in the office until mid-March to wind down the Practice and to transfer records to your new pediatrician. Please use our email address to request any medical records.  Our email is info@doc4kids.com. Since we have so many records now to transfer, we ask that you give us 10 working days to process and send the records.

We will be closing the phone lines down shortly and the above email address will soon be the only way to contact us. If you need medical records, please fill out a medical release form found here. We will need an actual signature before forwarding to your new doctor. As a courtesy, we do not charge for records sent to your next doctor; however, if you want personal copies, there is a fee for this.  The fee is dependent on the size of the file.
