Case Management

The case management team at CHKD is staffed by registered nurses and masters prepared social workers. Case managers work closely with patients and their families, as well as other members of the medical team to ensure optimal health care outcomes, access to services, and appropriate use of resources.

A case manager will be assigned to support you and your family throughout your child’s hospital stay. From evaluating your child’s needs at admission to coordinating a discharge plan, your case manager will help your family navigate the healthcare system.

How do case managers help?

  • Case managers will assess a child’s needs during admission. This includes care coordination and discharge planning.
  • Case managers will work with your child’s medical team regarding plan of care options.
  • Case managers may create and connect families to a discharge plan that documents a safe and effective discharge plan for your child, including post-care needs and services, referrals, rehabilitation, and more depending on the complexity of care coordination that is required.
  • Case managers may also connect you to your insurance provider. You case manager will work with your insurance company to advocate for the appropriate services needed when your child is discharged from the hospital.
