Referral Guidelines

Our practice provides evaluation, diagnosis, management and treatment of rheumatologic disorders in patients from birth through 21 years of age. We accept new patients under 18 years of age.

How can I refer my patient to CHKD's rheumatology practice?

When will my patient be able to obtain an appointment?

Your patient's estimated wait time for an appointment is approximately 4 weeks, but could vary depending on location and availability.

What work-up is needed prior to the first appointment?

Please provide us with the following information: reason why the patient is being referred; immunization record; relevant lab tests; x-rays; and physician notes that are not accessible through CHKD's Electronic Health Record. This information can be faxed to (757) 668-7784.

Where will my patient be seen?

Rheumatology offices are located at CHKD (Norfolk), Oakbrooke (Chesapeake), Tech Center (Newport News), and Landstown (Virginia Beach).
