Referral Guidelines
The Adolescent and Young Adult Medicine Clinic sees patients between the ages of 12 and 21. Referrals should be placed for “Adolescent Medicine” at our Norfolk, Suffolk and/or Virginia Beach location. Other patients may be seen on a case-by-case basis depending on their medical condition.
Please review our referral guidelines below to help ensure that your child/patient receives the best possible care.
- Non-urgent referrals (providers outside of KD): fax referral requests to 757-668-7721.
- Urgent and non-urgent referral questions by providers can be made through Doctor’s Direct: 757-668-9999 or by calling 757-668-7444 and selecting the provider line option in the phone tree.
- Families should schedule by calling 757-668-7444.
If your patient's referral is urgent, please contact the office directly for scheduling. Same day appointments may be available if schedule permits, no walk in appointments. Providers may contact office to discuss with staff about appropriate referrals.
Note: Same day appointments require prior insurance authorization. If insurance is unable to be verified, families will have to sign a waiver accepting financially responsibility if unable to verify active insurance; this makes them self-pay. We do offer a 30% discount for uninsured patients if paid in full at time of service.
Please provide us with the reason for referral, a list of current medications, and list of additional referrals if relevant. We also need any relevant office notes, labs, and immunization records that are not accessible through CHKD's Electronic Health Record (EHR).
Adolescent Medicine offices are located at Fort Norfolk (Norfolk), CHDK Main Campus (Norfolk), and Landstown (Virginia Beach).
Why Adolescent Medicine?
Adolescent Specialists are uniquely trained to assist adolescents with a variety of conditions and that may develop in the teen and young adult period such as GYN/reproductive health and sexuality concerns, drug, alcohol and tobacco prevention and counseling, psychosocial problems such as anxiety, depression, and self-esteem concerns, as well as acne and certain other skin conditions. We also work to help families and patients understand and embrace the dynamic during this new period of exploration, growth, and self-discovery.
